In-Office Membership Plans
For patients without dental insurance, our membership plans offer a simple annual option that bundles your preventive visits for the year at a discounted rate and includes discounts on additional treatment. Memberships are paid annually and are valid for 12 months from the date of purchase. We offer 3 membership plans:
1) Adult Membership Plan
- Patients over 14 years old without periodontal disease.
$440 per year cash or check
Includes:
2 Prophylaxis (cleaning) visits
2 Periodic oral cancer exams
4 bitewing x-rays
1 Emergency exam per membership year
2 Fluoride treatments
*15%/12% discount on all treatments including FMX/PAN
2) Periodontal Maintenance Membership Plan
$730 per year cash or check
Includes:
3 Periodontal Maintenance visits per membership year
2 Periodic oral cancer exams
4 bitewings or vertical bitewings as needed
Laser Bacterial Reduction as needed
1 emergency exam per membership year
2 Fluoride treatments
*15%/12% discount on all treatments including FMX/PAN and Scaling and Root Planning Therapy
3) Child Membership Plan
(Children under age 14)
$415 per year cash or check
Includes:
2 prophylaxis (cleaning) visits
2 periodic oral exams
Bitewing X-rays as indicated
2 Fluoride treatments
1 emergency exam per membership year
*15/12% discount on all services including FMX and PAN
*15% discount is for cash/check payments only.
12% discount are for credit card payments.
Membership Common Questions
Must be paid in full day of service to receive discount.
No additional discounts apply.
Any patient WITHOUT dental insurance can purchase a membership.
Membership IS NOT insurance.
Take-home products are not included.
All dental care must be provided by Scheske Family Dental— referrals not included.
Membership benefits are not transferable to another person or office.
It is the responsibility of the member to schedule and keep all appointments.
Membership is good for one year from the date of purchase.
All benefits must be used within the membership time period.
Unused services CANNOT be carried over.
Membership payments are non-refundable.
