Membership Plan

  • In-Office Membership Plans

    For patients without dental insurance, our membership plans offer a simple annual option that bundles your preventive visits for the year at a discounted rate and includes discounts on additional treatment. Memberships are paid annually and are valid for 12 months from the date of purchase. We offer 3 membership plans:

     

     

    1) Adult Membership Plan

     

    - Patients over 14 years old without periodontal disease.

    $440 per year cash or check

    Includes:

    2 Prophylaxis (cleaning) visits

    2 Periodic oral cancer exams

    4 bitewing x-rays

    1 Emergency exam per membership year

    2 Fluoride treatments

    *15%/12% discount on all treatments including FMX/PAN
